11/8/2006 Machine malfunction TX Hart InterCivic Medina County. 420 people lost the opportunity to vote in the US Senate race, which was "inadvertently omitted" from some ballots (presumably on the eSlate). Story Archive

Officials said the machines weren't calibrated for the 7am starting time, so the the Personal Electronic Ballots (PEB) used to activate the machines didn't work. The problem, according to Broward Supervisor of Elections spokeswoman Mary Cooney, was a voting system technician who activated machines at one precinct with the cartridge for the other. Both precincts are housed at the teen center. Elections officials resolved the Deerfield Beach problem by bringing in new voting machines and new cartridges. The technician also was replaced. 11/7/2006 Machine malfunction IN MicroVote Delaware County. MicroVote touch screen machines weren't operating at the start of the day in 75 precincts. Delaware County Clerk Karen Wenger says start cards that activate the machines for voters were programmed incorrectly by the company that installed the software. 11/17/06 update. MicroVote supplies the machines and technical servics, programs the machines, tabulates the results, and even counts the provisional ballots. The County is considering "whether county election officials should program and tabulate vote results." Story

11/7/2006 Machine malfunction MO ESS St. Louis County. Missourians for Honest Elections received news of two incidents in which ES&S iVotronic touch screen machines incorrectly recorded voters' intentions. These occurred at the Brentwood Community Center in St. Louis County. There have also been several verified reports from voters in St. Louis County who had this happen to them when trying vote absentee at the St. Louis County Board of Elections in Maplewood. Story

Diebold touch screens weren't working, and voters were told they would have to vote provisional ballots, which aren't counted until 10 days after the election. 11/7/2006 Machine malfunction PA Sequoia York County. A ninety-one year old woman who voted straight Democratic was surprised to find that the Sequoia Edge review screen showed she had cast a vote for Republican Rick Santorum. She was able to correct the ballot before casting it. 15 to 20 voting machines malfunctioned in the county: wires came loose inside some machines; others had been programmed with the wrong date.
